The STAR reporting system is used to address violations of the Code of Conduct in off-ice situations. We take all incidents seriously, but when an incident occurs it is sometimes easy to overact. For that reason we ask you to observe the 24-hour rule and wait until the next day to place your report. Please try and fill in all the pertinent information in as much detail as possible, so we can easily follow up on the situation. If we receive a report that does not come under the jurisdiction of STAR we will route it to the appropriate authority.

The STAR report is NOT to be used to report any violations of the Sexual Abuse, Physical Abuse, Emotional Abuse, Bullying, Threats or Harassment, Hazing or any suspicions or allegations of child physical or sexual abuse. Please use the SafeSport incident report in those cases.

Under the incident reporting system, you will be "registering" your incident in our system so that we can make an adequate review.

GUIDELINES

Generally issues should follow this protocol.
Issues regarding coaches should be reported to the D4 Coaching Director.
Issues regarding referees should be reported to the D4 Referee in Chief.
Issues regarding off-ice officials should be reported to the D4 Referee in Chief.
Issues regarding the rink staff/management should be reported to the rink owners for off-ice incidents.
Issues regarding players should be reported to the Association/Club the player skates out of.

What should be reported? How?

If it is an issue where a coach lost his temper as a father (not on the ice, during a game, or team event) then this would be considered a STAR report (D4 STAR Coordinator responsibile to review the outcome).  If it is a situation that occurred during a team event - then it would be a coaching issue (D4 Coaching Director to review the outcome).  If it is a bullying, hazing or other serious violation then it would be part of SafeSport and would be handled by the D4 SafeSport Coordinator.
